Social Worker – Part Time

We are pleased to offer the following exciting new opportunity: Social Worker – Part Time Maplestone Enhanced Care is a licensed and accredited nursing home located on Main Street in Halifax, Nova Scotia and is home to 87 clients requiring nursing home care.

The care and service address the unique needs of every individual while promoting flexibility and independence.

Reporting to the General Manager, the Social Worker provides clinical social work assessment, treatment and case management to clients and/or families in the facility.

This is a part-time opportunity with approximately 48 hours bi-weekly.

We must also work together to maintain a safe, healthy and home-like setting for all clients and fellow employees.

The successful candidate will be primarily responsible to: Provides counseling and therapy to clients and families as appropriate and develops support groups as necessary; provides therapeutic crisis intervention as required; Facilitates client care conferences, team meetings and family meetings; participates as a member of the interdisciplinary team; Conducts tours of the facility and interviews prospective clients and/or families through home, hospital and/or office visits to obtain pre-admission information; Chairs the Admissions Committee; answers inquiries from family members and general public on admission procedures; screens and assesses client applications for admission; Assists with the admission, transfer (to other units), and discharge of clients in coordination with the interdisciplinary team; Prepares funding requests and reports relating to client needs; maintains client records as required; Develops and maintains relationships with Department of Health, community institutions and community groups as necessary; Provides in-services and education sessions to clients and staff on a variety of topics; Ensures client safety by following and applying all standard operational procedures, safe work practices and training provided.

Qualifications for the role include: Bachelor of Social Work Current registration with the provincial association of social workers; Minimum of one-year of clinical experience; “Great People Striving for Great Things” is one of the pillars of our Strategic Plan.
